Carolyn has been a resident of California since 1982, relocating from Milwaukee, WI. She developed her sales and marketing career in the technology industry and has more than 30 years of experience in sales development and execution, commercialization and market launch of leading-edge technologies. Her combined expertise includes executive team management and leadership, strategic planning, P&L management, marketing, tactical sales, client relationship management.

In 2003, Carolyn pursued her great passion for Interior Design and founded CAZ Designs Inc, now celebrating 17 successful years, providing architectural design services to residential and commercial clients throughout southern California.

Carolyn’s community involvement began in 2006 serving on the Board of Directors for Olive Crest Homes and Services. She recruited volunteers to support fundraising events and chaired the Annual Tommy Bahama Golf Tournament from 2008 – 2014. She also Co-Chaired their Annual Gala from 2006-2008. In 2011, she committed to yearly donations supporting St. Jude Children’s Research Hospital as well as volunteering and leading the design committee for the Annual St. Jude OC Gala from 2013 – 2016. In 2013, she aligned with Lung Cancer Foundation of America, dedicated to raise both the funds and the national profile of lung cancer in order to substantially increase support of innovative and groundbreaking research efforts. She became a volunteer and Chair of the silent auction committee supporting the Annual Day at the Races fundraising event from 2014 – 2017. She Chaired the 2018 and 2019 DATR Planning Committee and joined the Board of Directors on March 1, 2019. Carolyn also serves on the Board of Directors for StandUp For Kids (SUFK) who operate several programs with the same end goal of leading homeless youth to self-sufficiency and preventing at-risk youth from gang involvement, substance abuse, teen pregnancy, and dropping out of high school. Carolyn joined the SUFK Board of Directors in October 2018 and has led the design committee for the SUFK Annual Gala from 2018-2020.

An attorney and lung cancer survivor, David was diagnosed 22 years after he quit smoking and a little over one year after he climbed Mt. Kilimanjaro. He underwent surgery for removal of two lobes of his right lung. Like many people, after his diagnosis, David became acutely aware that lung cancer lung cancer does not receive the research funding commensurate with its toll on public health. As a result, David became a fervent patient advocate, originally as the Lung SPORE representative at UCLA, where he met LCFA co-founders Kim Norris and Lori Monroe. David was then invited to serve on the United States Department of Defense’s Congressionally Directed Medical Research Programs’ Integration Panel, and was the sole patient advocate at the table for the groundbreaking National Lung Screening Trial’s Data and Safety Monitoring Board (DSMB). David has also petitioned lawmakers in Washington, DC, in order to increase funding for lung cancer research. David lives in Minnesota.

David Carbone graduated summa cum laude from Amherst College in 1977 and received an MD and a PhD in Molecular Biology and Genetics at Johns Hopkins University in 1985. He then did an Internal Medicine internship and residency at The Johns Hopkins Hospital through 1988 followed by a Medical Oncology fellowship at the National Cancer Institute in Bethesda, MD. In 1991 he was appointed Assistant Professor at the University of Texas Southwestern Medical Center and was promoted to Associate Professor with tenure in 1995.

He was recruited to Vanderbilt University in 1996 where he was promoted to full Professor in 1998. He served as Professor of Medicine, Cell Biology, and Cancer Biology at the Vanderbilt University Medical Center and Director of the Experimental Therapeutics Program and the Thoracic Oncology Center at the Vanderbilt-Ingram Cancer Center. He was also Director and Principal Investigator of the Vanderbilt Specialized Program of Research Excellence (SPORE) in Lung Cancer and the Strategic Partnering to Evaluate Cancer Signatures UO1 consortium.

In 2012, Dr. Carbone joined Ohio State’s Comprehensive Cancer Center – James Cancer Hospital and Solove Research Institute. He is a professor in the Division of Medical Oncology and will be developing and leading a new thoracic oncology center. The thoracic oncology center will bring physicians and scientists together to develop new and targeted approaches to treating and curing lung cancer. Dr. Carbone’s research interests, grant support and publications have been focused on lung cancer, and specifically proteomic and expression array signature development, lung cancer genetics, cancer immunotherapy, tumorassociated immunosuppression mechanisms and gene therapy.

Recent research directions include molecular profiling of lung cancers and preneoplasias, especially the use of mass spectrometry-based proteomics. He has over 150 peer-reviewed publications and review articles, has served on several NCI grant review panels, including the clinical program project parent committee, and has continuous NCI funding since early in his career. He has served on organizing committees for both ASCO and AACR and is currently Chair of the Lung Biology subcommittee for the Eastern Cooperative Oncology Group and currently serves on the Board of Scientific Counselors for the NCI.  Currently, he is President of the IASLC through 2017.

Dr. David H. Johnson is the Donald W. Seldin Distinguished Chair in Internal Medicine and Chairman of the Department of Internal Medicine at UT Southwestern Medical Center in Dallas, Texas. He is a board certified medical oncologist who specializes in experimental therapeutics and the treatment of lung cancers. From 1983 to 2010 he was a member of the faculty at the Vanderbilt University Medical School where he held the Cornelius A. Craig Chair of Medical and Surgical Oncology and served as the Director of the Division of Hematology and Medical Oncology and Deputy Director of the Vanderbilt-Ingram Cancer Center of which he was a founding member. In 2010 he relocated to Dallas Texas to assume his current position.

In 2004-2005 Dr. Johnson served as the President of the American Society of Clinical Oncology (ASCO) during which time he worked to advance ASCO’s cancer survivorship and quality care initiatives. From 2005-2009 he was a member of the International Association for the Study of Lung Cancer (IASLC) Board of Directors. Additional past activities include Chairman of the American Board of Internal Medicine (ABIM) Board of Directors, Chairman of the ABIM Medical Oncology Subspecialty Board, Chairman of the Thoracic Committee of the Eastern Cooperative Oncology Group, member of the FDA’s Oncology Drug Advisory Committee and service on the Boards of Directors of the National Comprehensive Cancer Network and the LiveSTRONG Foundation. He has authored more than 400 journal articles and 40 book chapters and edited four oncology textbooks.

Dr. Johnson is an elected member of the Association of American Physicians, a Master of the American College of Physicians and the recipient of Established Investigator Award from the Cancer Research and Prevention Institute of Texas. In 2016 he received the Distinguished Achievement Award from the American Society of Clinical Oncology. He earned his medical degree from the Medical College of Georgia and received his Medical Oncology training at Vanderbilt University.

Donna Whitney, Director of Digital Strategy and Web at Lung Cancer Foundation of America, brings 20 years of digital technology expertise and a decade of nonprofit healthcare experience to her role. Previously Senior Manager of Digital Strategy at National Stroke Association, Donna combines technical innovation with strategic vision to advance healthcare advocacy through digital solutions, driving the organization's mission to fund lung cancer research. 

David Levinson is the Founder and Executive Director of Big Sunday. On Big Sunday Weekends, more than 50,000 volunteers of all ages worked together at more than 500 different nonprofit sites in 69 different towns and cities throughout Southern California. David and Big Sunday have been honored by many organizations, including The Religious Action Center, The Returned Peace Corps Volunteers, The Asian-Pacific Women’s Center, The Western Center of Law and Poverty, and The Association of Fundraising Professionals. Last spring, David was named Best Nonprofit Leader in California by California Volunteers, Governor Schwarznegger and Maria Shriver. His book, The Help Book; A Reluctant Volunteer’s Guide to Helping Out, Pitching In, and Giving Back, has been published by Hudson Street Press, a division of Viking/ Penguin. David has also written movies, plays, television shows, novels, newspaper articles, theme park attractions, and advertising. He recently co-wrote the pilot Mercy for Sony Pictures Television.

David was glad to become involved with LCFA not long after his father, Mark, passed away following a long and hard battle against lung cancer. David’s father was a former smoker who had quit smoking more than twenty years before his diagnosis. Mark, a lawyer, was a great father and husband, and a wonderful, wise and kind friend to everyone who was lucky enough to know him. David will tell anyone who will listen that as far as he’s concerned, it doesn’t matter how or why anyone develops lung cancer – if it’s someone you love, you just want it cured. David lives in Los Angeles with his wife, Ellie Herman, and their kids Rebecca, Jack, and Izzie.
